How Insulin was introduced to the World? 1922 at the University of Toronto, the scientists
went to a hospital ward with diabetic children, most of them comatose and dying from diabetic
keto-acidosis. This is known as one of
medicine’s most incredible moments. Imagine a room full of parents sitting at
the bedside waiting for the inevitable death of their
child. The scientists went from bed-to-bed and injected
the children with the new purified extract – insulin. As they began to inject the last comatose
child, the first child injected began to awaken. Then one by one, all the children awoke from
their diabetic comas. A room of death and gloom, became a place
of joy and hope. Thanks Dr.
Banting and Dr Best!

Can pancreatic cancer cause diabetes? If we are diabetics will our diabetes worsen
because of pancreatic cancer? Watch this video about diabetes in pancreatic
cancer! You will find all the answers to these questions
and many more! Part of the pancreas is dedicated to producing
hormones (which are substances that are poured into the blood and exert their effect throughout
the body). Specifically, it produces two hormones, insulin
and glucagon, which are responsible for controlling the metabolism of sugars in the body. The reflection of the state of this metabolism
is the level of glucose in the blood. When the blood glucose is very high, the pancreas
will produce insulin, which makes blood glucose enter into the cells and decrease its blood
concentration. Conversely, if there is too little glucose
in the blood, the pancreas will produce glucagon, which forces the liver to release its glucose
reserve into the blood, increasing its concentration, then normalizing it. That is the normal situation. But when a cancer appears in the pancreas,
the production of these hormones suffers due to two phenomena: First phenomenon: Pancreatic cancer directly destroys part of
the gland responsible for producing insulin and glucagon, since it compresses and crushes
it as it grows. That is why the production of insulin and
glucagon will be progressively lower. Second phenomenon: Pancreatic cancer prevents the pancreatic
enzymes from going to the intestine. Because of this, these enzymes begin to destroy
the pancreas itself. When the damage to the gland is established
by these chemicals that digest proteins, the production of insulin and glucagon is further
diminished because there is less pancreatic tissue to produce them. So, if we were not yet diabetics, we will
be. And if we were already type II diabetics,
the type of diabetes in which the patient still has insulin reserve in the pancreas,
the control of blood glucose levels will worsen, and we will be more likely to have elevated
blood glucose levels.
[Music] in these lectures we’re going to talk about diabetes mellitus diabetes mellitus can be defined as a chronic metabolic disease characterized by elevated plasma glucose this is usually due to insulin deficiency in the case of type one diabetes type one diabetes can be immune mediated due to beta cell destruction or idiopathic which is non immune mediated this particular type is very rare acquired diabetes is diffuse damage of the beta cells with subsequent insulin deficiency and this can occur from infections or drugs that affect the pancreas impaired action of insulin secondary to insulin resistance is the hallmark of tannic 2 diabetes probably the most common form of diabetes that we encounter in medical practice one can also get the combination of these two abnormalities pre-diabetes is defined as elevated kazmo glucose levels below the diagnostic criteria for diabetes but above the normal range gestational diabetes or pregnant but pregnancy induced diabetes is any degree of glucose intolerance with onset or first recognition during pregnancy the definition applies where the insulin or only diet modification is used for treatment and whether or not the condition persists after pregnancy elevated plasma glucose can be defined as a plasma glucose level that occurs between the range of 100 and 125 milligrams per deciliter also on a glucose tolerance test a range of plasma glucose between 140 and 199 milligrams per deciliter this is usually two hours after a 75 gram oral glucose load and then finally a glycosylated hemoglobin level or hemoglobin a1c of 5.7 26.4% criteria for diagnosing diabetes are numerous and this but this particular series of slides will take you through differentiating the normal range pre-diabetes and over diabetes mellitus a random plasma glucose in the setting of classic hypoglycemic symptoms such as polyuria polydipsia polyphagia plus a plasma glucose level that exceeds or is equal to 200 milligrams per deciliter will give you the diagnosis of diabetes fasting plasma glucose after an eight hour fast within the normal range should be less than 100 milligram per deciliter pre-diabetes is defined as a fasting plasma glucose between 100 and 125 milligrams per deciliter and avert diabetes occurs when the fasting plasma glucose is greater than or equal to 126 milligrams per deciliter plasma glucose during a 2 hour 75 gram oral glucose tolerance test under normal circumstances should be less than 140 milligrams per deciliter in pre-diabetes this will range between 140 and 199 and in overt diabetes greater than 200 milligrams per deciliter hemoglobin a1c of less than 5.7 is normal between 5 point 7 and 6 point 4 indicates pre-diabetes and overt diabetes is diagnosed when the hemoglobin a1c exceeds 6.5 percent I referred earlier in the slide to classic hypoglycemic symptoms there is a generally accepted triad although they don’t necessarily always occur in patients with diabetes but more often than not excessive urination or polyuria excessive hunger polyphagia and excessive thirst or polydipsia are hallmarks of elevated serum glucose in the absence of hypoglycemic symptoms and abnormal fasting plasma glucose an abnormal oral glucose tolerance test test or hemoglobin a1c should be confirmed by repeated testing let’s say a few words about the metabolic syndrome this is a group of risk factors that increase the probability of developing tight diabetes mellitus and cardiovascular disease this in turn can result in impaired glucose metabolism central body obesity hypertension and hyperlipidemia there’s also an increased relative risk of developing cardiovascular disease and an increased risk of developing diabetes in fact the risk for cardiovascular disease is two times normal and the risk for developing of the diabetes is five times normal [Music]
Hello and welcome to my youtube channel podologie und mehr i am andrea Today it’s about diagnosing diabetes And then what will happen you’ll probably wonder what diabetes has to do with your feet In diabetes, there are following diseases that have a serious impact on the feet What is diabetes now actually diabetes is a metabolic disease Namely the pancreas which makes insulin so it insulin is Key that unlocks the cell so sugar from the blood comes into the cell and the cell with it So sugar will supply fuel if that key does not work properly Then, the blood sugar stays in the blood and it can not get into the cell anymore There are several types of diabetes type 1 and type 2 are the most common Type 1 diabetics mostly children and adolescents Meanwhile, also found that older people can get type 1 but really the most really have Type 2 diabetes that is the so-called sugar Among diabetes type 2 is one lifestyle disease are diabetes type 2 Although the plants are innate but you can do quite a lot with nutrition if People who are good at healthy food do not have much weight in sports the whole not so bad type 2 diabetes Back to the feet. a consequence of diabetes is the so-called polyneuropathy Pretty Latin word and called poly means a lot, neuro nerves are and Partie is the pathological damage us in this polyneuropathy Is it so that the nerves in your feet begin to die off Or not as conductive as they should be. that means diabetics It can happen that you have sensations in your feet And those sensations can be tingling, burning, feeling when standing on cardboard or if the foot is in cotton Of course, the whole thing leads to insecurity in diabetics and The next problem is if the nerves of course not The more the conductivity is transmitted, the more painful it is And it feels warm and cold that is when the pain is no longer passed on Then, diabetics come up with something spitzy A wound is on the foot, every normal would then limp or hobble. And watch what’s there. the diabetic just does not He just keeps running on this wound Unfortunately, I have seen it all too often that from small wounds really that will be huge wounds And these wounds if not professional Supply no longer approach And sometimes the feet also have to be amputated And so it does not get that far, for amputation Is actually thought that the podologist Really the feet regularly peeks the nails so it cuts no There is a risk of injury if the patient does it himself And if now there are any wounds in it or there are any foreign bodies in it Can the podiatrist say that there is something inside or go the foot ambulance let it handle. go to the wound manager, here is a wound this is properly supplied we podologists should be with reduce the doctor’s amputation rate and that minor is small and major is large amputations. So we should actually. All amputation types at the foot Reduce. So minor is almost a toe away, the forefoot away major amputations below knee and thigh amputation unfortunately happens too often Diabetes is a creeping disease that means it does not hurt Everyone who has diabetes will agree with me you do not really care about them if it does not hurt and that is the biggest problem many diabetics are diligent, take their Tablets but they control the blood sugar level not every day or only every three months at the doctor some neglect very much Many diabetics are diagnosed far too late they already have success diseases And when the therapy starts and the blood sugar level is set correctly. eg the daily measured sugar should be two hours after eating a frame for 110 to 140 And the long term sugar value should be within a range of Less than 6.5, that would be the optimum, many are on 6.5 to 7.5. ( in germany) These are the values that are determined by a blood loss that is the so-called long-term sugar value. the blood value hba1c Measured This is a subgroup of red blood cells where the sugar molecule adheres to the red blood cell, and then this hba1c arises And all the more so Sugar molecules on the blood particles just adhere the higher is this hba1c value a high hba1c worth means also the blood is just Thick it is more sugar inside what should actually be in the cells and not swim around in the blood because if He swims around in blood, it can unfortunately also come to clumping And in the smallest vessels and this diabetics have increased heart circulatory diseases, get just by too much blood sugar who in the family Diabetes should go to the blood check regularly let this hba1c value control, and of course on the nutrition pay attention to sport Good sport is pronounced a little hard One can also call it increased everyday movement that means instead of taking the lift up the stairs, not everything is driving my car So small things where it just bring more movement, everything is positive for the blood sugar level Short to the numbers diabetes is not a rare disease in germany Currently more than six million diabetics type 1 and type 2 and it’s really going to be annual 300,000 diabetics More (as of 2018) There is also a dark figure that is so pi times thumb over two million if, you, the whole have extrapolated in some Years, and neat pack what diabetics there is the podologen we should also provide So that just this amputation rate drops Of course, that’s not great when you lose a toe first and then in your forefoot and then unfortunately it often goes on The podologist looks that it does not happen. the sequelae of diabetes are not just polyneuropathy and the angiopathy there is also the Retinopathy and nephropathy of retinopathy is blind to diabetes And nephropathy is kidney damage what do you do if you just got the diagnosis of diabetes What is very important is to change diet and make increased everyday movement Lose weight who has too much on the ribs, but that is always said to be more obese with everyone He can do that, still only with really quarter Annually, to. Diabetologists should also check the hba1c value kidney doctor or check with the family doctor and the kidney values At least once a year we see the optician go to the ophthalmologist to give me this retinopathy Check if you have any damage or not And the better you take care of your diabetes the longer It takes until you get the following illnesses The further you can do that Pushing the back there are even those who get the never following diseases Because it is just as far as you have set in diabetes well and thus get along well Did you just ask her to write down the comments below Click here to subscribe to the next video and do not forget to click bell And we’ll definitely see each other in the next video andrea bye

In this video, we will learn about what
insulin does in our body and how does it act. So, let’s begin. Insulin is an anabolic
hormone with multiple synthetic and growth promoting effects which we will know later on. It acts through tyrosine kinase linked insulin receptor. Insulin receptor is a tetrameric
protein. It has two alpha chains which lies outside the cell membrane and two beta chains
which traverses the cell membrane. Alpha chains have insulin binding domain. Binding of insulin
on alpha chain causes auto-phosphorylation of beta chain This leads to activation of
tyrosine kinase linked with the receptor. This enzyme phosphorylates intracellular proteins including family of insulin receptor substrates which includes IRS-1 to IRS-4 and GAB-1 which in turn, activate multiple signaling pathways including PI3K and MAP kinase pathways which mediate the metabolic and mitogenic activities of insulin on cells. Insulin signaling also
facilitates trafficking and docking of vesicles containing GLUT-4 to the plasma membrane which causes increased glucose uptake into cells. This GLUT-4 mediated uptake of glucose occurs specially in fat cells, hepatocytes and muscle cells In muscle cell, increased glucose uptake
leads to glycogen synthesis. Insulin also causes increased amino acid uptake, activates transcription and translation to cause protein synthesis. It inhibits protein degradation
as well. This accounts for growth promoting effect of insulin. Increased uptake of glucose
in hepatocytes stimulates glycogenesis. We already have adequate amount of glucose in
body. So, insulin inhibits new glucose formation from glycogen and other substrates like amino
acids. Excess of glucose is converted into fatty acid which is transported to circulation as triglycerides in very low density lipoprotein. Insulin stimulates lipoprotein lipase which converts triglycerides into fatty acid which is uptaken into adipocytes. Glucose in adipocytes
is converted to glycerol phosphate which becomes a source of glycerol that combines with fatty
acids to form tryglyceride and is stored in adipocytes as fat. Insulin inhibits hormone
sensitive lipase and prevents lipolysis. Thanks for watching. Subscribe the channel for more
